To get the delayed birth certificate in Assam follow the following steps:

Step 1: Firstly you must know who can apply for duplicate birth certificate so they are-

  • When the applicant had original birth certificate
  • When a person does not have an original birth certificate
  • When he / she has never applied for a birth certificate
  • When the applicant has lost or misplaced the original certificate

Step 2: Then, you need to fill the application form with following details-

  • Your complete name
  • The name of your parents, including first and last names. In the case of your mother’s name, it will be her maiden name before marriage
  • Your exact date of birth
  • The address of your parents
  • The place of birth where you were born

Step 4: After which you need to visit the municipal corporation office of the city where you were born

Step 5: Then pay the fee for the issuing the copy of the birth certificate. The fee may varies according to State/city

Step 6: When you pay the fee, you will receive an application form which is needed to be filled with all the details

Step 7: After this submit the application form to the officer and then you will be issued a receipt and will be instructed when you can get the copy

How to apply for Online Application Form in Assam?

  • Firstly visit the official website of Assam that is
  • Then go to “List of Available Online Services ” from the drop down list of “Online Services”
  • Now select “Issuance of Birth Certificate” and enter applicant details, address details, New born details: date of birth, gender, name of father, mother, place of birth, house address, information name, address of parents at time of birth, Birth weight
  • After that enter new born parents details: mother location, village or town of mother, permanent address, religion of family, other religion name, education and occupation of father, mother, age of mother at time of marriage and child birth, type of deliver, method of delivery, duration of pregnancy
  • When the complete form is filled submit it and take its print out
