Who Can Apply for a Correction?

  • If your age is at least 18 years old than the applicant whose name is on the certificate can apply.
  • If your age is less than 18 years old than the Parents or legal guardians whose name is on the certificate can apply.

You have to submit your current signed photo identification and it should not be expired. If your birth certificate contains both father and mother record than signature of both and their identification is necessary. If you are above 18 years, you can submit your own identification photocopy.

Types of correction in changing names

#1, Misspelled name

If the name on your birth certificate has a spelling mistake than you don’t need to have court order. You have to provide the required documents (listed below) to show the correct spelling.

How to Change?

You have to submit photocopies of current, signed photo identification and a check or money order to be made payable to the municipal authority along with one of the following documents:

  • Parent’s passport. or
  • Parent’s marriage record .or
  • Parent’s naturalization certificate that should have been naturalized prior to the birth of the child. or
  • Birth certificate of an older brother or sister

#2. Missing name

If the missing name is to be added on the birth certificate, these can be added without a court order. To add the first name and if the last name is also wrong to the name you already using, than while adding the first name last name will be updated. You must provide the required documents (listed below) to show the name that are in use.

How to Change?

You must submit photocopies of current, signed photo identification and a check or money order for the fee to be made payable and one of the following documents:

  • Your correct name, date of birth and parental information should be correct in the apology letter from the hospital where you were born stating that they are responsible for the error or
  • Immunization records or
  • Your First census proof in use after birth or census taken at least before 10 years or
  • Letter from physician including treatment dates that should be from the last 10 years or
  • Date of admission in school proof or
  • Official school record or
  • Any Religious document or
  • Your life insurance policy

#3. Changing Parentage or Surname

Changing a name is a legal procedure and requires a court order to be approved. You have to provide proof that the information on your birth certificate is not correct.

How to Change?

You have to submit the documents mention below to the Registrar of Births and Deaths in your Municipality

  • Your parents marital proof with all the information correctly mentioned in it.
  • Any of your documents of hospital, where the name of your parents and your date of birth is correct like discharge paper etc…

In addition to these proofs you will also need to provide the following –

  • An affidavit made on a Rs.100 Stamp Paper, duly attested by a Class I Magistrate
  • Copies of ‘change of name’ advertisement published in two local newspapers
  • Newspapers have a standard format for publishing change of name
  • You can also have the change of name published in the Government Press in your State. The same will then be published in the gazette, copies of which you will need to attach with your application
  • You will also need to fill in a requisite form (that can be collected from the Municipal office at a small fee) for correction of name.

The Department of Health and Mental Hygiene’s Division of Vital Records (DVR) handles all the issues related to birth certificate. You can bring documents to DVR in person or you can mail them.

To apply for a correction by mail

  • Complete and sign the birth certificate correction application. You have to download the application.
  • Enclose copies of identification indicated on the form.
  • Enclose original documents to support the correction + 1 copy of each of these documents.
  • Calculate the fees and enclose your payment
  • Include a self-addressed, stamped envelope.
  • Mail your correction application package to the address below.
  • Note that standard first-class U.S. postal service mail is not tracked.
